Exosomes Explained: What They Are, What's Legal in the UK, and Why I Use Purasomes

Exosomes are one of the most talked about words in aesthetics right now, and also one of the most misused. Patients ask me about "exosome facials" they have seen advertised, sometimes with no explanation of where the product actually comes from or how it is applied. As an NHS GP and aesthetic doctor in Hale, I want to explain what exosomes actually are, what the evidence currently shows, and a question that matters more than most patients realise: is what you are being offered even legal in the UK.

What Are Exosomes?

Exosomes are tiny extracellular vesicles, roughly 30 to 150 nanometres across, released naturally by cells. Think of them as microscopic delivery parcels, carrying proteins, lipids and genetic signals between cells. They play a genuine role in inflammation, tissue repair and matrix regulation throughout the body, which is why researchers across dermatology, wound care and regenerative medicine have become interested in them.

The important thing to understand is that "exosomes" is a description of a type of particle, not a single product. Exosomes can be derived from human cells, animal cells or plants, and can be applied to the body in very different ways. Those two variables, source and delivery method, are what actually determine whether a treatment is safe, evidenced and legal, far more than the word "exosome" on a treatment menu.

What Does the Evidence Show?

Current research on exosomes in skin rejuvenation is genuinely encouraging, though still developing. Systematic reviews have found consistent improvements in skin elasticity, hydration and wrinkle depth, with molecular evidence showing increased collagen and elastin synthesis in treated skin. Exosomes also appear to support wound healing and calm inflammation, which is part of why they are often paired with procedures that briefly disrupt the skin barrier.

The evidence is promising rather than settled, which is exactly why the source and safety profile of the product matter as much as the results it promises.

Most published studies are still early stage, with varied products, dosing and outcome measures. That is a normal part of any emerging treatment area, but it is also why I only use products with real manufacturing transparency and a clear safety record, rather than choosing based on marketing claims alone.

The UK Legal Picture: Topical vs Injected

This is the part most patients are never told, and it matters more than any ingredient list. The MHRA, the UK's medicines regulator, treats injected exosomes as medicinal products. That means injecting exosomes, whether into the skin, under the skin, or into a vein, is not currently permitted for aesthetic use in the UK, regardless of where the exosomes came from.

Separately, exosomes derived from human tissue are banned for cosmetic use in the UK entirely, due to safety, disease transmission and ethical sourcing concerns. If a clinic describes their product as human derived or as "stem cell exosomes from donor tissue", that is not a legal cosmetic treatment.

If you are ever considering an exosome treatment, three questions are worth asking before you book: what is the source of the product, is it applied topically or injected, and can the clinic show you product documentation. A clinic confident in what it is offering will answer all three without hesitation.

Why I Use Purasomes

In my clinic, I use Purasomes, made by the Italian company Dermoaroma. Purasomes are derived from organic bovine colostrum, the nutrient-rich first milk produced by cows, rather than from human tissue, which is part of why they can be legally and ethically used in UK aesthetic practice. Dermoaroma collect the colostrum only after calves have received their share, and it goes through a patented purification process called AMPLEX Plus to isolate the exosomes and remove unwanted proteins and impurities.

Each treatment I use contains roughly 10 billion purified exosomes alongside 20 growth factors, including cytokines and signalling proteins that support cellular activity and collagen production. In my clinic, Purasomes is always applied topically, immediately after a treatment such as medical microneedling that temporarily opens the skin's surface, never injected. Is It Safe?

Topical, non-human-derived exosome products such as Purasomes have a favourable safety profile in published data, and applying them topically avoids the additional risks that come with injecting any biological product. As with any treatment that follows microneedling or laser, mild redness or flaking for a few days is normal. Choosing a doctor led clinic that can show you exactly what product you are receiving and how it is sourced remains the most important safety step, more so than any single ingredient.
