The under eye area is one of the first places many people notice ageing, tiredness or a change in skin quality.

Patients often tell me the same thing. They feel well, they are sleeping reasonably, they are looking after themselves, but they still look tired. Friends ask if they are exhausted. Photographs look less fresh. Makeup starts sitting differently under the eyes. The skin looks thinner, darker, more creased or less bright than it used to.

I do not believe the under eye area should be treated with a one size fits all approach. Dark circles, eye bags, hollowing, crepey skin and tired looking eyes can all have different causes, and the safest treatment depends on understanding what is actually happening beneath the surface.

As an NHS GP and aesthetic doctor with nearly 20 years of medical experience, I assess the anatomy, skin quality, lifestyle factors, medical history and treatment goals before recommending any under eye treatment. The aim is not to make you look different. The aim is to help you look fresher, healthier and more rested, while still looking completely like yourself.

Why the under eye area changes with age

The skin beneath the eyes is some of the thinnest skin on the face. It has less underlying support than many other areas and is closely affected by changes in collagen, volume, circulation, pigmentation and facial structure.

This is why the under eye area can change earlier than the cheeks, jawline or lower face. Even small changes can look noticeable because the eye area is central to facial expression and communication.

For some people, the main issue is dark colouring. For others, it is hollowing or shadowing. Some patients have true puffiness or eye bags. Others have fine lines, crepey skin and reduced elasticity. Many people have a combination.

The most important step is identifying which of these factors is driving the concern.

Common causes of dark circles and tired looking eyes

Dark circles are not one single diagnosis. This is one of the biggest misconceptions in under eye treatment.

Some patients have darker pigmentation in the skin itself. This may be genetic, more common in certain skin types, or linked with inflammation, eczema, rubbing or allergies.

Some patients have visible blood vessels beneath thin translucent skin. This can create a blue, purple or grey tone.

Some patients have hollowing, where volume loss or facial structure creates a shadow under the lower eyelid. This can make the under eye look darker even when the skin colour itself has not changed.

Some patients have puffiness or eye bags, which can cast a shadow underneath and make the area look more tired.

Others notice their under eye area worsens with poor sleep, dehydration, stress, alcohol, hay fever, sinus congestion or hormonal changes.

This is why a treatment that works beautifully for one person may do very little for another. The correct treatment depends on the cause.

Under eye concerns we commonly assess in clinic

Dark circles

Dark circles may be caused by pigmentation, visible blood vessels, shadowing from hollowing, puffiness, skin thinning or a combination of factors. They are one of the most common reasons patients search for under eye treatment in Hale and Altrincham.

Treatment depends on the dominant cause. If the darkness is mainly due to pigmentation, the plan may involve skincare, pigment control and realistic expectations. If it is mainly due to thin skin and poor skin quality, regenerative treatments may be more appropriate. If it is mainly due to shadowing, we need to assess the structure of the tear trough and midface carefully.

Crepey under eye skin

Crepey under eye skin often develops as collagen and elastin gradually decline. The skin may look finely lined, loose, dehydrated or less smooth. Makeup may crease, concealer may sit in lines, and the skin may look older than the rest of the face.

This is where skin quality treatments such as polynucleotides, NCTF skin boosters, carefully selected microneedling and medical grade eye skincare may be considered.

Hollow under eyes

Hollowing beneath the eyes can create a sunken or shadowed appearance. Some people have this from a young age because of facial structure. Others notice it more with age, weight loss, hormonal change or midface volume loss.

Hollowing is not always best treated directly under the eye. Sometimes the surrounding cheek and midface support need to be considered. Sometimes the safest plan is not to inject the tear trough at all.

Eye bags and puffiness

True eye bags are often caused by lower eyelid fat pads, fluid retention, lymphatic factors, allergies, sleep, genetics or changes in lower eyelid support.

Non surgical treatments can sometimes improve the surrounding skin quality, but they cannot reliably remove true fat pads or significant lower eyelid bags. In some cases, the most honest advice is to consider an oculoplastic or surgical opinion rather than spending money on treatments that will not address the main problem.

Fine lines around the eyes

Fine lines may be due to skin thinning, sun damage, dehydration, collagen loss and repeated facial movement. Under eye skin quality treatments may help improve texture, hydration and resilience, while lateral eye lines may need a different assessment.

The treatment plan should be tailored to the type of line, the quality of the skin and the patient’s expression pattern.

My diagnosis first approach

I do not start by asking, “Which treatment do you want?”

I start by asking, “What is causing this concern?”

During consultation, I assess the under eye skin, lower eyelid support, cheek structure, degree of hollowing, skin texture, pigmentation, puffiness, medical history and lifestyle factors. I also ask about allergies, eczema, hay fever, sleep, weight change, medication, previous filler and previous under eye treatments.

This matters because the under eye area is delicate. It is not a place for guesswork.

A patient with thin crepey skin needs a different plan from someone with true lower eyelid fat pads. A patient with pigmentation needs different expectations from someone with shadowing. A patient with previous tear trough filler and swelling needs a completely different conversation from someone who has never had treatment before.

The safest and most natural results come from choosing the right treatment for the right patient.

Polynucleotides for under eyes in Hale

Polynucleotides are one of my preferred options for patients whose main concern is under eye skin quality.

They are injectable regenerative treatments made from purified DNA fragments. Depending on the product, these may be fish derived, so allergy history is important. Polynucleotides are designed to support the skin environment, improve hydration, encourage tissue repair processes and gradually improve the appearance of thin, tired or crepey skin.

For the under eye area, they are not used like filler. They do not create volume in the same way and they are not intended to fill a hollow tear trough. Instead, the aim is to improve the quality of the delicate skin over time.

Polynucleotides may be considered for:

  • thin under eye skin

  • crepey texture

  • fine lines

  • poor skin quality

  • tired looking eyes

  • mild dark circles linked to skin thinning

  • patients wanting a gradual regenerative approach

Results are not instant. Most patients require a course of treatments, and improvement tends to develop gradually over weeks to months. The effect is usually subtle, natural and progressive rather than dramatic overnight change.

I particularly like polynucleotides for patients who want a fresher under eye appearance but are not suitable for tear trough filler, or do not want the risks and potential puffiness associated with filler in this delicate area.

NCTF for under eyes

NCTF is a skin booster containing hyaluronic acid together with a complex of vitamins, amino acids, minerals, coenzymes and antioxidants designed to support skin quality.

In the under eye area, I use NCTF as a skin quality and hydration treatment rather than a filler. It is not designed to create structure or replace lost volume. Its role is to improve the appearance of tired, dull, dehydrated or finely lined skin.

NCTF may be suitable for patients who describe their under eyes as:

  • dull

  • dehydrated

  • thin

  • tired

  • creased

  • lacking brightness

  • showing early signs of ageing

It can be particularly helpful when the concern is not one single deep hollow, but more of an overall tiredness or loss of luminosity.

As with all under eye treatments, technique and patient selection matter. Some patients are more prone to swelling, and the under eye area must be treated conservatively.

Polynucleotides vs NCTF under eyes

Polynucleotides and NCTF are often discussed together, but they are not the same treatment.

Polynucleotides are usually chosen when the focus is regenerative skin repair, thin skin, crepiness and longer term skin quality. NCTF is often chosen when the focus is hydration, brightness, luminosity and overall skin freshness.

Most patints’s benefit from both combined.

Others may not be suitable for either, particularly if the main issue is a significant eye bag, festoon, medical cause of puffiness or a surgical lower eyelid concern.

The consultation determines the best plan.

Microneedling and exosomes around the eye area

For selected patients, medical microneedling may be considered as part of a wider skin quality plan. Microneedling creates controlled micro injury in the skin, which can support collagen remodelling and improve texture over time.

Around the eye area, treatment must be careful and appropriate to the anatomy. The lower eyelid skin is delicate, so treatment depth, device choice and product choice matter.

In most some cases, microneedling may be combined with an exosome based regenerative complex. Exosomes are an emerging area within aesthetic medicine and skin science. They are used topically after procedures such as microneedling to support the skin environment and recovery signals.

I would describe exosomes as promising, but still emerging. They should not be presented as a guaranteed solution for dark circles or eye bags. They may be considered for selected patients wanting overall skin quality improvement, particularly where the wider face also shows dullness, texture change or loss of radiance.

Medical grade skincare for under eye ageing

Under eye treatment should not only be injectable.

The right skincare can make a meaningful difference, especially for fine lines, texture, dryness and pigmentation. However, the eye area is more reactive than the rest of the face, so stronger active ingredients must be used carefully.

Depending on the patient, I may discuss medical grade eye products, gentle retinoid strategies, pigment supporting ingredients, hydration, barrier repair and daily SPF.

Sun protection is particularly important. UV exposure accelerates collagen breakdown and pigmentation, both of which can worsen the appearance of dark circles and crepey skin.

Skincare will not remove true eye bags or replace lost volume, but it can support healthier, more resilient skin and improve the results of in clinic treatments.

Why I do not routinely offer tear trough filler

Many patients ask about tear trough filler because they have seen dramatic before and after photos online.

Tear trough filler can sometimes produce excellent results in carefully selected patients. However, the under eye area is one of the most technically challenging areas to treat with filler. It is also one of the areas where poor patient selection can lead to long lasting problems.

Potential issues include puffiness, swelling, lumpiness, blue or grey discolouration, asymmetry, migration, persistent malar oedema and an unnatural heavy under eye appearance.

For this reason, I do not routinely offer tear trough filler in my Hale clinic.

That is not because filler is always wrong. It is because I believe many patients are better served by a more conservative, skin quality focused and regenerative approach. I would rather say no to a treatment than risk creating a problem that is difficult to reverse.

My philosophy is simple: the under eye area should look fresher, not obviously treated.

A typical patient story

A woman in her forties attends clinic because she feels her under eyes make her look permanently tired. She sleeps well, exercises and uses good skincare, but the skin under her eyes has become thinner and more creased over the last few years.

She has considered tear trough filler, but she is worried about puffiness and looking overdone.

Patients in their forties often describe looking more tired despite feeling well. Perimenopause, stress, poor sleep, facial volume change and collagen loss can all become more visible around the eyes.

Patients in their fifties and beyond may have a combination of skin thinning, puffiness, lower eyelid laxity, pigmentation and structural hollowing. Treatment may still help selected concerns, but expectations and suitability become even more important.
