Sculptra vs Radiesse: Which Collagen Stimulator Is Right for You?
One of the biggest changes I have seen in aesthetic medicine over recent years is that more patients are asking for treatments that do something beyond adding volume. I hear this almost weekly:
I don't really want filler. I want something that improves my own collagen.
That is usually when the conversation turns to collagen stimulators, or biostimulators. The two I use for this are Sculptra and Radiesse.
Both stimulate collagen. Both improve aspects of facial ageing. Both continue to create change after you leave the clinic. And neither behaves like a conventional hyaluronic acid filler.
But they are not the same treatment, and they are not interchangeable. I use them for different patients and different goals. So if you have been researching which is "better", the more useful question is what are we actually trying to improve?

What Is a Collagen Stimulator?
As we age we gradually lose collagen, facial volume and structural support. Traditional hyaluronic acid fillers restore volume or alter proportions by placing a gel within specific tissues.
Collagen stimulators work differently. They provoke a controlled tissue response that encourages the body to produce and remodel components of its own extracellular matrix, including collagen. The result is therefore not solely dependent on what is placed into the face on treatment day. Part of the improvement develops gradually afterwards.
This is why you see them described as biostimulators. But grouping them under one umbrella makes them sound far more similar than they are. Sculptra contains poly-L-lactic acid. Radiesse contains calcium hydroxylapatite. Their physical properties differ, their biology differs, and I use them differently.
How They Actually Work, and Why That Matters
This is the part most comparison articles skip, and it is the part that actually explains the difference.

That difference in mechanism is not academic. It is why the two products behave differently in the tissue, why they have different risk profiles, and why one gives you something to look at on the day and the other does not (Sickles et al.; Aguilera et al.).
What Is Sculptra?
Sculptra contains microscopic particles of poly-L-lactic acid which stimulate a gradual tissue response and new collagen formation.
The most important thing to understand is that the immediate fullness you see after treatment is not the final result. Sculptra is reconstituted with sterile water, and that water plus normal post treatment swelling temporarily makes the face look fuller. It settles. The genuine result then develops over the following weeks and months as collagen remodelling takes place.
How the product is prepared genuinely matters. Dilution volumes of seven millilitres or more are associated with lower nodule risk, and a 2025 study found that allowing reconstituted PLLA to stand for twenty four to seventy two hours before injecting produced measurably greater collagen formation than injecting it immediately. Your vial should be prepared days before your appointment, not minutes before.
So Sculptra is a treatment for someone happy to be patient. It is not designed to give you a sculpted jawline before you leave the clinic. I reach for it when I am looking at broader facial ageing. Read more about Sculptra at the clinic here.
What Is Radiesse?
Radiesse contains calcium hydroxylapatite microspheres in a gel carrier, and that gives it two useful properties at once. Used structurally, the gel provides immediate support and contour. The microspheres then stimulate tissue remodelling and collagen production afterwards.
That makes it particularly useful when I want structure now and biostimulation later, typically around the chin, jawline, angle of the jaw and pre jowl region.
Radiesse can also be diluted or hyperdiluted, and this changes its purpose entirely. Rather than using it for projection, we distribute it more diffusely to focus on skin quality, firmness and collagen stimulation. So even within Radiesse there are two quite different conversations. The Main Difference, in One Line Each
Sculptra is for gradual, broader restoration across the face. Radiesse is for targeted structural support combined with collagen stimulation. Hyperdilute Radiesse is for when the priority shifts from structure to skin quality.
That is not a rigid rule, because aesthetic medicine is more nuanced than that. But it is a useful starting point.
Which Is Better For What?
Broader facial ageing
This is where I will usually consider Sculptra. A typical patient says something like "I don't need anything particularly filled, my whole face just seems to have lost something." The temples have hollowed. The lateral cheeks have flattened. There is early jowling. The overall shape of the face feels different from ten years ago.
That is not one line or hollow needing filled. It is a broader change in facial support, and Sculptra suits it because we can treat selected areas more globally and let improvement develop gradually.
A 2024 systematic review of PLLA in facial application, rated Level of Evidence I and covering twenty studies, concluded that PLLA is a reasonably safe and effective facial injectable across different areas, depths and reconstitution methods, with most adverse events mild and self limiting (Xu et al.). A separate 2024 review reported improvements in dermal thickness, facial volume and aesthetic scores, while judging the overall quality of the underlying evidence to be relatively low (Signori et al.).
I think that balance matters. There is good clinical evidence supporting Sculptra. We do not need to call it miraculous for it to be a very useful treatment.
The jawline
When the problem is predominantly structure, I am more likely to consider Radiesse. Picture someone with reasonable overall facial volume but decreasing definition through the lower face, a slightly under projected chin, a pre jowl hollow disrupting the line between chin and jaw, or a posterior jawline that would benefit from support.
There I want a product I can place deliberately to create structure, and systematic review evidence supports CaHA for facial indications including the jawline and cheeks, with positive results around the chin and pre jowl area (Guida et al.; Amiri et al.).
But this does not mean everyone who dislikes their jawline needs Radiesse. If somebody already has a naturally wide or heavy lower face, adding product along the jaw makes the face broader rather than more lifted. And if the real problem is significant loose skin, structural product does not address what the patient is actually seeing.
Jowls
Sometimes Sculptra. Sometimes Radiesse. Sometimes neither.
A jowl does not develop because there is an empty space needing filled. It develops through loss of structural support, changes in facial fat, skin laxity and soft tissue descent together. If early jowling is part of general deflation and collagen loss, Sculptra may make sense. If poor chin projection or a pre jowl hollow is exaggerating the lower face, structural Radiesse may be better. If skin quality is contributing, that needs addressing separately.
And if somebody has substantial tissue descent and significant excess skin, neither treatment will reproduce what a facelift achieves.
Temples
Sculptra can be particularly useful for temple hollowing in appropriately selected patients. As facial volume changes, the temples become more concave, which makes the upper face look more skeletal and alters the transition between forehead, temple and cheek.
Rather than creating an obviously filled appearance, Sculptra allows gradual restoration through collagen stimulation. Patients often tell me a few months later that people keep asking whether they have been away, without anyone being able to say why. For most of my patients, that is exactly the result they want.
Crepey or lax skin
This is a different problem again. If the concern is thin skin, crepiness, loss of firmness, fine wrinkling and mild laxity rather than volume or structure, then hyperdilute Radiesse becomes the interesting option, because diluting shifts the aim from projection to tissue quality.
The evidence here is encouraging but not unlimited. A 2024 review of facial Radiesse specifically noted that high quality randomised evidence for diluted and hyperdiluted facial applications is still lacking despite their increasing clinical use (Guida et al.). That is worth knowing before anyone sells it to you as settled science.
Collagen stimulating does not mean non surgical facelift. Those are different claims, and only one of them is true.

Speed, Longevity and Number of Sessions
These three questions come up in every consultation, so here they are together.

Those duration figures come from a 2026 systematic review in Aesthetic Plastic Surgery that pooled fourteen studies of both agents and found significant improvements in skin elasticity, wrinkle reduction and facial volume for each (Ferreira et al.).
But these are not expiry dates. The treatment does not stay switched on for eighteen months and then vanish overnight. Your connective tissue is constantly remodelling, and ageing continues underneath a good result. I think about biostimulation as improving the trajectory, not stopping the process.
On sessions, I do not subscribe to rigid rules that every woman over a certain age needs a set number of vials. Your face is not a formula.
Does Sculptra Add Volume?
Yes, and this is commonly misunderstood. People hear that Sculptra is "not a filler" and assume it cannot create fullness. But collagen itself takes up space. As new collagen develops, areas that lost volume appear fuller and better supported. The difference is that the change comes from tissue remodelling rather than from injecting a gel.
For most patients that is exactly the appeal. They are not asking for bigger cheeks. They are asking for their face to look a little more like it used to.
Which Looks More Natural?
Either can look extremely natural. Either can look unnatural when used inappropriately. The product alone does not determine the result. Anatomical assessment, quantity, placement, technique and judgement all matter.
I do not think natural results simply come from injecting less. They come from putting the right amount of the right treatment in the right place. Sometimes a carefully placed structural treatment gives a far more natural result than repeatedly adding small amounts of soft filler across the face. And sometimes adding structure is exactly the wrong thing to do.
Are They Safer Than Filler?
I would not describe either that way. They are different products with different properties, and both remain injectable medical treatments with real risks.
Temporary effects include swelling, bruising, tenderness, redness and discomfort. Sculptra has recognised risks including papules and nodules, which is why dilution volume, injection depth and the massage protocol matter. CaHA can also produce nodules and irregularity. The 2026 systematic review found adverse effects were predominantly mild across both agents, but did record one case of compression necrosis with CaHA, which is a reminder that these are vascular risks in a vascular area (Ferreira et al.).
There is also one practical difference from hyaluronic acid filler that everybody should understand before consenting.
Neither Sculptra nor Radiesse can be dissolved with hyaluronidase. There is no reversal agent for either.
That single fact is the strongest argument for choosing a medically qualified injector for these particular treatments. Patient selection, anatomy, technique and experience matter enormously when the result cannot be undone.
Can You Have Sculptra and Radiesse Together?
Yes. They do not compete, and in some faces they are genuinely complementary.
Somebody might benefit from Sculptra for widespread collagen and volume loss while also having a structural deficit through the chin or jawline better addressed with Radiesse. Another patient might have Sculptra for the face and hyperdilute Radiesse for crepey neck skin.
That does not mean I would do everything at once. With collagen stimulators I particularly value staged treatment. Give the biology time to work. Assess the result. Then decide what is genuinely still needed.
What If I Don't Want Filler?
This is often what brings patients to collagen stimulators in the first place. But when someone tells me they do not want filler, I always explore what they actually mean. Usually it is one of these.

So Sculptra is not automatically the answer simply because somebody dislikes the idea of filler. Neither is Radiesse. Sometimes a small amount of carefully placed hyaluronic acid is the most precise and conservative option available. Sometimes the problem is mainly skin quality and we should be discussing microneedling, polynucleotides or skincare. And sometimes there is no treatment I would recommend at all.

Because Sculptra is usually a course of two sessions and structural Radiesse is often a single session, the headline price is not the comparison that matters. Your actual plan and total cost are agreed at consultation, in writing, before anything is booked.
So Which One Would I Choose?
If you came into my clinic tomorrow, I would not choose based on your age, and I would not expect you to have decided before you arrived. I would look at your facial proportions, where volume has changed, where structural support has been lost, your skin quality, whether the lower face is naturally light or heavy, the degree of laxity, your previous treatments, and above all what you actually want to improve.
Broader facial volume and collagen loss with gradual restoration as the aim tends to point to Sculptra. Precise structural support through the chin or jawline tends to point to Radiesse. Crepey skin and declining tissue quality tends to point to hyperdilute Radiesse.
And sometimes I choose neither. There is no prize for using the most fashionable injectable.
The best treatment is the one that makes sense for the face sitting in front of me.
Frequently Asked Questions
Which is better, Sculptra or Radiesse?
Neither is better in the abstract. Sculptra suits broad collagen and volume loss where the aim is gradual restoration. Radiesse suits targeted structural support at the chin and jawline where you want immediate correction as well as collagen stimulation. Choosing between them is a diagnostic decision, not a preference.
Which lasts longer, Sculptra or Radiesse?
Pooled data reports effects lasting up to around 25 months for PLLA and around 12 to 18 months for CaHA. These are averages across heterogeneous studies rather than guarantees, and results vary by individual, area, quantity and number of sessions.
Can you have Sculptra and Radiesse at the same time?
Yes, and they are often complementary. I usually prefer to stage them rather than doing everything in one appointment, so that the collagen response from the first treatment can be properly assessed before deciding what else is genuinely needed.
Is Sculptra a filler?
Not in the conventional sense. It does not add volume by placing a gel. But new collagen occupies space, so treated areas do look fuller and better supported over time. Saying it is "not a filler" is true about the mechanism and misleading about the outcome.
Can Sculptra or Radiesse be dissolved?
No. Unlike hyaluronic acid filler, neither has a reversal agent. This is the single most important practical difference and the strongest reason to choose an experienced medical injector for these treatments.
Which one is better for the jawline?
Usually Radiesse, because it allows deliberate structural placement at the chin, pre jowl area and jaw angle with an immediate result. The exception is when the jawline looks less defined because the whole face has deflated, in which case Sculptra may address the actual cause.
How much do Sculptra and Radiesse cost in Hale and Altrincham?
Sculptra is £495 per vial and £950 for two. Radiesse is £395 for one syringe, rising to £1,050 for three. Hyperdilute Radiesse is £450 per area. Consultation is £75 and is redeemable against treatment.
How soon will I see results?
With structural Radiesse, immediately, with the appearance evolving as the carrier resorbs and remodelling develops. With Sculptra, the initial fullness settles first and genuine change appears gradually from around six weeks, continuing to develop for roughly three months after your final session.
Sculptra and Radiesse in Hale, Altrincham and Cheshire
