  • Treating lips in isolation while ignoring perioral support and ageing changes

What Is Lip Filler Migration?

Migration is when filler sits beyond where we intended, commonly causing:

  • a blurred top-lip border

  • a “shelf” or mound above the upper lip

  • loss of crisp definition

  • distortion of shape over time

Migration isn’t inevitable. In my experience, risk is higher when lips are repeatedly topped up without a plan, when product choice doesn’t match tissue, or when placement is too superficial.

  • whether the upper lip is rolling inward

  • symmetry (and what is normal asymmetry)

  • perioral support and smile dynamics

  • your previous filler history (if any)

2) The right product for the tissue

Soft, mobile products can be better suited to dynamic areas. I choose product characteristics to match:

  • skin thickness

  • movement

  • how much support is actually needed

3) Conservative volumes (especially for first treatments)

Natural lips are rarely built in one sitting. I’d much rather do:

  • a subtle first treatment, then

  • reassess once settled
    …than overfill and try to “fix” it later.

4) Correct depth and placement

Superficial placement can increase risks of:

  • visible filler

  • blurring of the border

  • lumpiness

  • the “shelf” effect

5) A sensible top-up philosophy

Over-topping-up is one of the biggest drivers of the overfilled look. If we do review/tweak, it’s based on your anatomy once everything has settled—not on a “routine schedule”.

6) Being willing to say “not today”

Sometimes the best aesthetic decision is:

  • to stop,

  • to dissolve old migrated filler first, or

  • to treat surrounding support (chin, oral commissures, perioral lines) rather than forcing more into the lip.

Lip Filler Aftercare: What I Recommend

Your result is shaped by the first 1–2 weeks. Most people feel surprisingly normal, but temporary swelling and bruising are common.

What’s normal after treatment

  • mild–moderate swelling (often peaks at 24–48 hours)

  • tenderness

  • small bruises at injection points

  • temporary asymmetry while swelling settles

  • a slightly “full” feeling for a few days

For the first 24–48 hours

  • keep lips clean; avoid unnecessary touching

  • use a cool compress (wrapped) if swollen

  • sleep slightly elevated if you tend to puff up

  • avoid alcohol (can worsen bruising)

  • avoid strenuous exercise and overheating (sauna/steam/hot yoga)

For the first 2 weeks

  • avoid facial massages around the mouth unless advised

  • avoid aggressive skin treatments (peels/microneedling/strong actives) near the area

  • avoid very high heat exposure

  • keep lips hydrated with a simple balm

  • wear SPF around the mouth if exposed to daylight

Not unless we aim for that. My default is subtle restoration, softness, and balance—then reassess once settled.

How long does swelling last?

Most swelling improves in 3–7 days, with final refinement around 2 weeks.

Can you guarantee no migration?

No. Nobody can guarantee zero risk. Choosing the right product, conservative volumes, correct depth/placement, and sensible top-ups significantly reduces the likelihood.

How often should lips be topped up?

Less often than social media suggests. Over-topping-up is a common cause of migration and the “filler look.” I prefer review based on your tissues and how the filler has settled.

What if I’ve had filler before and I’m worried it’s migrated?

We can assess. Sometimes the best plan is to dissolve and restart conservatively, rather than adding more on top.

Is lip filler safe?

Hyaluronic acid fillers are widely used, but all injectable treatments carry risk. A medical clinic should have robust consent, aftercare and complication protocols.

Do you treat the lips in isolation?
